Folic Acid: A required component for the conversion of Homocysteine to methionine, Folic Acid supplementation reduces Homocysteine levels. In a randomized clinical trial, 150 patients with ischemic heart disease were given 1 of 5 doses of folic acid (200, 400, 600, 800, 1000 mcg/day) or placebo. Serum Homocysteine levels were measured after 3 months of supplement use and again 3 months after stopping supplements. Regardless of initial Homocysteine level, increased folic acid supplementation (up to 800 mcg) had increased Homocysteine lowering effects. Those with higher initial Homocysteine levels had greater lowering effects with Folic Acid. Low levels of daily Folic Acid intake have been associated with high serum Homocysteine levels, especially in the elderly.
Vitamin B12: B12 is considered by many to be the second most important nutrient, next to folic acid, in reducing Homocysteine levels. A required co-factor in the conversion of Homocysteine to methionine via the folate pathway, vitamin B12 deficiency is common in the elderly and those consuming vegetarian diets. Meta-analysis of Folic Acid and B12 reveal that Homocysteine levels are dropped, on average, about 25% by Folic Acid regimens and an additional 7% by the addition of B12 to the regimen. Another study confirmed the value of vitamin B12: Hemodialysis patients with hyperhomocystinemia had decreases of almost 20% in 4 weeks with B12 and Folic Acid with 1 mg each considered optimal.
Vitamin B6: Another important ingredient for Homocysteine metabolism, vitamin B6 is known to be deficient in the typical American diet, particularly the elderly. Even in low doses, vitamin B6 has been shown to lower Homocysteine levels in elderly patients- even among those previously supplemented with folic acid.
Betaine (TMG): Betaine (Trimethyglycine, or TMG) acts as a methyl donor to Homocysteine- converting it to methionine. This conversion pathway occurs in the liver and kidney and may contribute to Homocysteine reduction in some individuals with very high methionine, or after ingestion of high levels of methionine. We include Betaine in appropriate safe dosage for those people needing a more aggressive regimen. Sub-therapeutic levels are used to support the more ubiquitous vitamin approach.
